Please fill out the New Patient Review Request form.  Our new patient coordinator, will be calling you within 1-2 business days.

Fill out the necessary paperwork and have your spine imaging results ready to give to our new patient coordinator.

If your spine films are over a year old, then either have your primary care physician order new ones or make an appointment to see one of our physician assistants and we will order the appropriate films for you.  

Use our online contact form.  Many patients like this feature as we can respond to you quickly.

The main reason is to expedite your care.  Our physician assistants can perform an evaluation, obtain necessary spine imaging and communicate directly with our surgeons.  If spine surgery is necessary, we will have you follow-up with our surgeons.

We often order functional  (flexion/extension) xrays to evaluate for any spine instability that cannot be seen on a MRI.  When you have a MRI you lying down and motionless.

We do see patients who were inolved in a motor vehicle accident (MVA) on a case by case basis.  Our new patient coordinator will ask you to provide your personal injury protection information (PIP).  If your PIP is exhausted we can see you under your health insurance if we are a contracted provider.
